Medical history of a patient is crucial to treat chronic diseases such as cancer and diabetes. And to handle epidemic and pandemic-like situations where data exchange is required to treat a large number of patients, inter-operation of healthcare plays a key role. However the existing healthcare system is isolated and does not exchange information. To solve this problem, we have proposed a blockchain based secure interoperable healthcare system which is prone to both internal and external threats. We have connected healthcare in a distributed manner to protect from external threats and further divide it in distributed peer to protected internal threats. We have also proposed a peer level (PIM) and organization level (OrgIM) identity manager for efficient identity management and access control. The proposed system is evaluated in terms of transaction throughput, latency and send rate. Through simulation, we have observed 103.5% reading and 135.02% writing throughput improvement as compared to existing public blockchain based solution. As compared to permissioned blockchain based solution, we have improved 1.79% read and 32.6% writing throughput.
