Telecare Medical Information Systems (TMIS) have revolutionized remote healthcare delivery, offering significant advantages to patients, medical professionals, and the healthcare industry as a whole. However, existing security measures in TMIS, particularly regarding user authentication, are susceptible to various attacks. This paper proposes a novel decentralized authentication framework for TMIS that leverages Blockchain-based Account Bound Tokens (ABTs). The proposed system leverages blockchain’s immutability and decentralization to provide a secure and transparent authentication process between patients, doctors, and the medical server. The framework utilizes smart contracts to store user credentials and access control policies encoded as ABTs. During authentication, user credentials are verified against ABTs, ensuring only authorized users can access the TMIS system. The proposed protocol’s security is rigorously assessed using the formal security verification tool AVISPA (Automated Validation of Internet Security Protocols and Applications). Additionally, informal security analysis is employed to further validate the framework’s robustness. Performance evaluation demonstrates that the protocol is computationally efficient while upholding user privacy and security.
